BRITISH COLUMBIA, Canada (26 August 2007) -- According to a B.C. coroner's report, the scuba diving accident that killed Ronald McKindsey resulted from inexperience and faulty equipment. McKindsey was certified as a PADI Open Water diver and had only been diving about 20 times when he and his friends when diving near Denman Island two years ago on May 21, 2005.
