BRITISH COLUMBIA, Canada (26 August 2007) -- According to a B.C. coroner's report, the scuba diving accident that killed Ronald McKindsey resulted from inexperience and faulty equipment.

McKindsey was certified as a PADI Open Water diver and had only been diving about 20 times when he and his friends when diving near Denman Island two years ago on May 21, 2005.

The report cites the adjustment of McKindsey's second stage regulator, which made breathing difficult and resulted in sudden cardiac arrest.
