Police: Diving equipment failure killed Illinois scuba diver

PORT WASHINGTON, Wisconsin — The Ozaukee County sheriff's department said its investigation of a fatal scuba diving accident revealed that equipment failure likely killed the victim.

CDNN previously reported that 48-year-old Phil J. Patz, of Stone Park, Illinois, died while deep diving in Lake Michigan about 20 miles north of Milwaukee and about nine miles southeast of Port Washington.

About four hours after he failed to resurface, searchers found his body at a depth of about 91.5 meters (300 feet).
